Yes, and you upload them to the server the same way you would any other file. Be aware of the download problem: without a fast-download host, players pull custom maps from the game server itself at a crawl, and many give up at the loading screen. We do not currently offer fast downloads, so if your plan is a heavy custom-map rotation, tell us before you order and we will be straight with you about what the join experience looks like.
Twenty four is the traditional community server size and works well for Payload and Attack/Defend. Competitive formats are much smaller, at six or nine per side. Two things to watch: Versus Saxton Hale wants a full server to be any fun, and Mann vs Machine needs the server configured for thirty two slots or it refuses to start, so pick your slot count around the mode you actually intend to run.
Yes, if you want your server listed publicly. Valve requires a Game Server Login Token, which you generate for free against your own Steam account, and a server without one is limited to direct connections. Generate it under app ID 440. Several hosting guides tell you to use 222840, which is Left 4 Dead's server tool and will not work.
