Players without it stay in the Exiled Lands and lose nothing. On Legacy (UE4) servers, the older rule still applies: every player connecting to a Siptah-only server needs the Siptah DLC. Cosmetic and building DLC packs do not gate connections, only the map DLC matters.

A nightly restart is plenty for most servers and keeps memory usage healthy. Under Enhanced the memory footprint is higher than UE4, so restarts matter a touch more. If you're running a big mod list or a very active server, you can set more frequent restarts via Scheduled Tasks in the LOW.MS Control Panel.
